I expressly AUTHORIZE the processing of my personal data by the persons, for the purposes and in the terms described by PAN-AMERICAN LIFE DE COLOMBIA, COMPAÑÍA DE SEGUROS, S.A. . I also declare that I have read and accepted and that I know how to access the Company's Policy Manual for Personal Data, authorization clause and the privacy notice at www.palig.com . I authorize THE COMPANY to contact me by e-mail and to send me more information about the products that it sells, as well as to send me the present quotation.

AUTHORIZATION FOR PERSONAL DATA PROCESSING:

I. In order to obtain a quote, I freely provide my personal information for all pre-contractual and contractual purposes encompassed within the insurance activity.

II. PALIC has expressly notified me that:

1. PURPOSE OF PERSONAL DATA PROCESSING: My personal data will be managed by PALIC for the following purposes: i) Processing my quote request as the process for an eventual relationship as a financial consumer, debtor, contractual counterparty, and/or vendor. ii) The contract negotiation process with PALIC. iii) The implementation and compliance with the contract that may be executed. iv) Fraud prevention and control. v) Settlement and payment of Claims. vi) All matters related to the integral administration of the contract. vii) Controlling the compliance of the requirements to access the General System of Integral Social Security. viii) The development of technical-actuarial studies, statistics, surveys, market tendency analysis and, in general, technical insurance studies. ix) Submittal of information related to financial education, client satisfaction surveys, and insurance commercial offers, as well as other services inherent to the insurance activity. x) Implementation of surveys regarding services provided by PALIC. xiii) Control and prevention of money laundering and financing of terrorism.

2. Data processing may be done directly by PALIC or by others in charge of such processing if considered necessary.

3. DATA USERS: The information may be shared, transmitted, delivered, transferred or released for the purposes herein to: i) The legal entities that have the status of branches, subsidiaries, affiliated Companies, or PALIC headquarters. ii ) The operators necessary for the compliance of rights and obligations originated from the contracts executed with PALIC, such as: adjusters, call centers, investigators, assistance companies, outside counsel, among others. iii) The insurance intermediaries that participate in the process of execution, implementation, and termination of the insurance contract. iv) Those individuals with which PALIC may work with the purpose of executing coinsurance and reinsurance contracts. v) FASECOLDA, INVERFAS S.A., and INIF, which are the legal entities that manage databases for purposes of fraud control and prevention, risk selection, and control of requirement to access the General System of Integral Social Security, as well as the development of actuarial and statistical studies.

4. INTERNATIONAL TRANSMISSION OF DATA TO THIRD PARTY COUNTRIES: In certain situations, the international transfer of my information is necessary to comply with the purposes of the data processing.

5. SENSITIVE DATA: That the answers to questions that have been asked or will be asked of me on sensitive personal data, in accordance with the current legal definition, are optional, so I expressly authorize for the processing of my sensitive data, especially that related to health.

6. PERSONAL DATA OF MINORS: The answers to questions about minors are optional. Therefore, I express and declare that I have not been obligated to answer them.

7. RIGHTS OF THE OWNER OF THE INFORMATION: As the owner of the information, we are entitled to the rights provided for in Law 1266 from 2008. In particular, the rights to be aware of, update, and revise the information about us that may have been collected.

8. ENTITIES WHO ARE RESPONSIBLE AND INCHARGE OF DATA PROCESSING: PALIC is responsible for data processing and its contact information is provided in the section of this document titled “How to Contact Us”

AUTHORIZATION: I expressly AUTHORIZE the data processing and, if necessary, the national and international transmission thereof, by the persons, for the purposes, and under the terms stated herein. Additionally, I state that I am aware of and may access the Company’s Personal Data Policies Manual, Authorization Clause, and Privacy Notice on www.palig.com.

Your access and control over the information

You may choose to contact us at any time. Also, you may do the following at any time by contacting us via email at servicioalclienteco@palig.com:

Change or revise any information we have about you.
Ask us to delete any information we have about you.
State any concern you may have about our usage of your information.

Security

We take certain precautions in order to protect your information. When you submit confidential information through the website, your information is protected both online and offline.
